Catalyst Physical Therapy is a preferred in-network provider for most major insurance plans, including Medicare. Our staff will contact your insurance carrier to verify benefits and eligibility associated with your plan. We will work with your physician’s office and insurance company to obtain the necessary documentation for billing your insurance company. Patients are responsible for co-pays at the time of service.
